Objectives
At the end of this unit the student must demonstrate:
· Knowledge and understanding of the growing health needs related with chronic conditions, impact on health services and respective orientations in health planning, both nationally and internationally;
· Knowledge and understanding about the theoretical models of self-management and respective intervention guidelines, for patients with chronic condition or at the risk of developing one, and respective carers;
· Knowledge and understanding about the educational models, and respective intervention guidelines, to promote the ability of the client/family/carers, in the self-management of chronic conditions;
· Ability to critically analyse the evaluation strategies, tools and action plans for a collaborative intervention with the client/family and carers;
· Ability to critically analyse intervention models used for the promotion of self-management in different chronic conditions;
· Ability to understand the experience of chronicity of the client and/or family/carers, identifying their problems and defining objectives;
· Ability to develop, apply, manage and evaluate the effectiveness of the intervention of physiotherapy, and/or inter-disciplinary team, to the self management of a chronic condition, and/or respective family /carers;
· Ability to implement an autonomous and reflexive practice for the promotion of chronic conditions, according to the ethical and deontological principles of the profession.

· Chronic disease in Portugal and internationally: individual, family, economical, political and social impact
· Theoretical Models of self-management and behavioural change
· Educational Models and respective strategies, which facilitate education
· Principles of client collaboration
· Evaluation and intervention models for the self management of chronic conditions.

Teaching method
The methods used are focused in the students, respecting their professional interests and individual differences. There is a theoretical approach with seminars and lectures. Tutorials are also used. The individual study, bibliographical search, community activities and peer discussion are also used.
Evaluation method
This unit is assessed through a written essay and respective oral presentation.
